In episode 1, death messages were turned off. Because of this, some causes of deaths are unclear.

12. Michael tried to swim in lava (then he spawned in his bed, which had lava above)
13. Michael tried to swim in lava (this time, his bed had burned up so he couldn't spawn in lava again)
